About Remote Patient Monitoring

Some chronic conditions, such as chronic obstructive pulmonary disease (COPD), congestive heart failure, diabetes and hypertension, need regular assessments to make sure you are on track and making progress with your treatment. Through remote patient monitoring, WVU Medicine can assess your health remotely while you are in your home.

Remote patient monitoring uses equipment and technology in your home to send information about your health to clinicians on our team. While in the program, you will have routine monitoring of vital signs and symptoms. You will also receive education about your condition on a regular basis. We engage with you and your provider to help when issues with your health arise.

WVU Medicine offers remote patient monitoring to assist in healthcare delivery. Remote home monitoring is available to all eligible patients in service by WVU Medicine. Contact your provider’s office if you are interested or would like more information.

How Does Remote Patient Monitoring Work?

After you enroll, your doctor will help you decide which devices to use. Then, we’ll send the healthcare monitoring devices directly to your home and help you get them set up.

You’ll also receive our SmartHub to connect your monitoring device(s) (not applicable for glucometers). The SmartHub device gathers all the measurements you collect and sends them to your doctor. Your measurements are automatically sent to your MyChart.

Remote patient monitoring may be helpful to you if you are a WVU Medicine patient and have one of these chronic conditions:

  • COPD
  • Diabetes
  • Heart Failure
  • Hypertension
  • Acute Covid
  • Long Haul Covid
  • Sepsis
  • Pneumonia
  • Atrial Fibrillation

We Provide Home Monitoring Equipment

Cellular or Wi-Fi service is required to use our equipment. Each device has its own network connection to send the information back to your care team. We provide training on how to use each device and will make sure you know what to do. Types of equipment we provide include:

  • Blood pressure monitor
  • Glucose monitor
  • Heart rate monitor
  • Pulse oximeter
  • Scale
  • Thermometer
