This includes any internships, externships, clinical rotations, observations, practicum, and/or capstone experiences. This information is not applicable to MD, DO students. Please contact Morgan Hart at [email protected] or 304-766-3525 for information regarding MD or DO students.
Student Guide Checklist
Step 1: The process for requesting clinical experiences varies according to type:
For rotations, observations, and/or capstone experiences ONLY (i.e. local programs sending groups of students): school will request; students in these groups are not to place requests independently. Please download the Contract CW Request form, found in the references below and email to: [email protected] once filled
out. It is essential that you ensure names, DOB, address, emails and all other information is correct on the form submitted, or it will create significant account and log-in information.
All other internship, externship, and practicum placements (including non-NP graduate nursing programs) are the responsibility of the student and/or school to obtain. Once a preceptor meeting the program requirements has been identified by the school/student, please complete the Clinical Request Form in the references below.
Step 2: An academic affiliation agreement (AA) must exist between our hospitals and the specific program at your institution prior to any clinical/non-clinical experience. Particularly if you are coming individually from a program that does not regularly send students, see that an AA exists and does not expire prior to the end of your scheduled experience. Contact your institution for more information.
*Note: This process can take months, so start early if an AA is not in place or is due to expire.
Step 3: Read and complete the following documentation.
- Completion of onboarding modules via learning management system (Workday).
- Documentation of current and proper:
- Immunization per CDC recommendations for healthcare workers.
- Influenza vaccine during flu season (required annually).
- PPD or chest x-ray with negative result (within previous 12 months); chest x-ray only required for positive PPD.
- Criminal Background Check.
- For Clinical Students/Instructors: AHA HCP BLS CPR Certification – Current.
- For Clinical Instructors Only: Current WV license confirmed and all other requirements above.
- Urine Drug Screen with negative result.
Step 4: You will receive an email with confirmation that you are cleared to start. It is required that you wear your school-issued badge while on campus.
Instructor Guide Checklist
Step 1: The process for requesting clinical experiences and ensuring an affiliation agreement (AA) should take place in advance of your requested start date. Please coordinate with your program to see that these items are completed.
Step 2: Read and complete the following documentation.
- Complete onboarding modules via learning management system (Workday)
- Provide documentation of current and proper:
- Immunization per CDC recommendations for healthcare workers.
- Influenza vaccine during flu season (required annually).
- PPD or chest x-ray with negative result (within previous 12 months); chest x-ray only required for positive PPD.
- Criminal Background Check.
- For Clinical Students/Instructors: AHA HCP BLS CPR Certification – Current.
- For Clinical Instructors Only: Current WV license confirmed and all other requirements above.
- Urine Drug Screen with negative result.
Step 3: Follow instructions provided on the School Information section to obtain any additional accesses as deemed necessary.
Step 4: Email [email protected] to schedule any unit-specific orientation requirements in advance of your first clinical day. Orient according to the Faculty Unit Orientation Checklist available below.
Step 5: Once you have received confirmation that you are cleared for clinical, the next step is to obtain a hospital-issued ID Badge (available to instructors only). It is required that you wear both your school and hospital-issued badges while on campus in your capacity as an instructor; if you are an employee with WVU Medicine, you cannot wear your employee badge while operating as an instructor.
School Guide Checklist
All items must be completed PRIOR to student/instructor arrival for requested experience:
Step 1: Student experience requested and approved.
Step 2: Academic Affiliation Agreement (AA) in place.
Step 3: Certificate of (Liability) Insurance (COI) sent.
Step 4: Students and instructors (coming on-site) oriented according to the appropriate guides.
Step 5: All documentation submitted according to packet.
Step 6: Instructor provided or sent for ID badge according to packet.
Step 7: Unit-specific orientation completed/sent (for clinical instructors only).
Step 8: CLEARANCE TO START received (via email) each semester.
