Resident Life

A day in the life of a Resident:

Our residency program is designed to provide residents with a structured clinical experience that
balances inpatient medicine, outpatient continuity, specialty clinics, documentation, patient follow-up, and
protected educational time—while supporting resident wellness.

7:00 am — Day Begins: Sign-Out
The inpatient day team receives sign-out from the night team, reviewing overnight events, new admissions, active issues, and important clinical updates.

7:30–9:00 am — Morning Patient Care
Inpatient residents:

  • Round on their patients.
  • Review overnight events and morning laboratory results.
  • Perform chart review.
  • Begin progress notes and develop the day’s care plans.

 

8:00 am — Outpatient Clinics Begin
Outpatient residents begin their morning continuity and specialty clinics, providing longitudinal patient care while developing expertise in both general internal medicine and specialty medicine.

9:00–10:00 am — Table Rounds
Residents meet with the attending physician for table rounds. The team reviews each patient’s clinical status, discusses diagnostic and therapeutic plans, and prioritizes the day’s patient-care needs.

10:00 am–12:00 pm — Attending Rounds
The inpatient team rounds together at the bedside. Residents present their patients, participate in clinical decision-making, communicate with patients and families, and work with the attending to establish individualized care plans.

12:00–1:00 pm — Lunch
Residents have designated time to take lunch.

Wednesday: 12:00–4:00 pm is protected didactic teaching time.

Friday: Grand Rounds at 12:00 pm is protected educational time.

1:00 pm — Afternoon Clinics Begin
Outpatient residents begin afternoon continuity and specialty clinics.

1:00–2:00 pm — Inpatient Documentation
Inpatient residents complete progress notes and update documentation following morning rounds.

2:00–5:00 pm — Patient Follow-Up, Admissions & Teaching
The afternoon provides flexibility for inpatient residents to:

  • Follow up on patients after morning rounds.
  • Respond to clinical changes.
  • Complete admission evaluations and documentation.
  • Coordinate care with consultants and the multidisciplinary team.
  • Participate in additional bedside or case-based teaching with the attending.

 

5:00 pm — Outpatient Clinics Conclude/Inpatient Sign-Out
Outpatient continuity and specialty clinics conclude. Residents should complete their clinical documentation by the end of the workday.

The inpatient night team arrives. The day team provides sign-out regarding active patients, pending studies, anticipated clinical issues, and patients who may require overnight attention.

5:30 pm–7:00 am — Night Float
The night float team assumes responsibility for the inpatient service. Night residents evaluate new admissions, respond to overnight clinical needs, and continue caring for patients already admitted to the hospital.

Our schedule is designed to provide residents with meaningful clinical responsibility while maintaining a sustainable approach to residency training. Residents have defined clinical responsibilities, protected educational time, opportunities for outpatient continuity, and a structured transition between day and night teams.

The goal is to develop confident, compassionate, and highly skilled internists while creating a residency environment where residents can learn, care for patients, and thrive.

Electives

  • Vacation Time: There will be four weeks of vacation per PGY year. Vacations will not be permitted during inpatient rotations.
  • Elective Options: Non-Medical Critical Care, Anesthesia with an emphasis on local and regional, Procedure Service, Multidisciplinary Bedside Ultrasound, Radiology/Imaging, Subspecialty Rotation
  • Possible Electives: Residents may choose to complete their elective experiences in any of the experiences offered at Thomas Memorial Hospital. Residents may also request additional elective options that require prior approval from the Program Director.
  • During clinic blocks, residents may have the opportunity to participate in various elective rotations including addiction medicine, hospice and palliative medicine, dermatology, office gynecology/women’s health, office ENT, office orthopedics/musculoskeletal, urgent care, emergency medicine, office ophthalmology, adolescent medicine, education rotation, allergy, nutrition and obesity, rehabilitation, pain management, and geriatric medicine.
  • All residents will be required to complete experience in Addiction Medicine and Hospice and Palliative Medicine during the program. For Addiction Medicine experiences, residents will undergo longitudinal experiences that will be integrated into their Wards and Emergency Medicine rotations, and potentially in their outpatient clinics as well.   • Morning Clinic: 8:00 am – 11:30 am
  • Afternoon Clinic: 1:00 pm – 4:30 pm

 

(Initial Visits 60 minutes, well visits 30 minutes, and acute visits 15 minutes)

  • Night Float Residents will be excused from continuity clinic
  • Closed on holidays
