To obtain medical records, you may now request your records by using the records request tool, through MyWVUChart, email, mail, or calling 304-598-4110 (or toll free 844-484-0304) Monday through Friday from 8 am – 4 pm.

Discharge

All patients will receive a personalized After Visit Summary at the time of discharge. This summary includes detailed instructions tailored to each patient’s individual care needs. We encourage all patients to follow up with their primary care provider after discharge from inpatient care. For any additional questions or concerns, please contact the nursing department.

Post-Visit Care

In the event of a medical emergency, patients should call 911 immediately.

Upon discharge, all patients will receive a personalized After Visit Summary, which includes comprehensive discharge instructions tailored to their individual care needs. This document serves as a guide to help support the transition from hospital to home and outlines any recommended medications, follow-up appointments, and self-care instructions.

Patients are strongly encouraged to schedule a follow-up appointment with their primary care provider to ensure continuity of care and support their recovery. Ongoing care may also include referrals to specialists, home health services, outpatient therapy, or community resources as needed.

For any additional questions or concerns following discharge, patients are welcome to contact the nursing department for assistance.

The DAISY Award program is a nationally recognized award program. Harrison Community Hospital has joined the growing network of more than 2,000 healthcare organizations across the country that currently participate in the award program created by the Daisy Foundation. The organization’s mission is to recognize the extraordinary, compassionate care that nurses provide to patients and families every day.

The DAISY Foundation is a not-for-profit organization, established in memory of J. Patrick Barnes, by members of his family. Patrick died at the age of 33 in late 1999 from complications of Idiopathic Thrombocytopenic Purpura (ITP), a little known but not uncommon auto-immune disease. (DAISY is an acronym for Diseases Attacking the Immune System.) The care Patrick and his family received from nurses while he was ill inspired this unique means of thanking nurses for making a profound difference in the lives of their patients and patient families.
Nurses may be nominated by patients, families, and co-workers.
