Voluntary NonOpioid Directive
In compliance with Article 54 Opioid Reduction Act, Summersville Regional Medical Center
offers patients the right to opt out of being prescribed an opioid medication by completing a
Voluntary NonOpioid Advanced Directive. The form shall indicate to a health care practitioner
that an individual may not be administered or offered a prescription or medication order for an
opioid. The advanced directive will be filed in the individual’s medical record and will be
transferred with the individual from one practitioner to another or from this facility to another
care facility in the event of an inter-facility transport.
Patients may access to form by clicking here. The Office of Drug Control Policy
encourages patients to complete the form in consultation with their primary care provider or
SUD treatment provider. However, consultation is not necessary to the validity of the form.
