Discharge

Planning for discharge begins at the time of admission. A patient’s Care Team works with them to develop a transition plan to fit their needs. Care Management (social workers and discharge planners) help arrange for any services needed after leaving the hospital, including home health services, transportation, durable medical equipment, placement, and medications. They can also provide information about other available resources in the community. Patients receive written instructions concerning medications, activities, appointments, etc. which is discussed with them by their nurses prior to discharge. Patients can call 855-WVU-Care (855-988-2273) to schedule follow-up appointments.

WVU Medicine St. Joseph’s Hospital has a system with Press Ganey that calls/emails our patients for survey questions post-visit. We also have programs where you can recognize specific staff members/nurses for any excellent care you received during your stay.

Caught You Caring Program

Staff, patients, and visitors can fill out a Caught You Caring nomination card for any staff member they want to recognize for showing kindness and compassion.

There are nomination cards located in clear holders throughout the hospital and clinics. Anyone can fill out a card and place it in any of the boxes located below the holders. You can also ask your nurse or any other staff member to help you with filling out a card.

The DAISY Award®

The DAISY Award® was created to honor and recognize nurses for the outstanding effort they make to provide extraordinary care every day. Staff, patients, and visitors can share their story of what their nurse meant to them.

There are nomination cards located in clear holders throughout the hospital and clinics. Anyone can fill out a card and place it in any of the boxes located below the holders. You can also ask your nurse or any other staff member to help you with filling out a card.

Support Groups

  • Breast Cancer Support Group
    • Offered to all breast cancer patients and survivors.
    • This group meets the third Thursday of each month, from 6 pm-7:30 pm at St. Joseph’s Hospital in Salvati Hall. You can enter through the side door to the right of the main entrance.
    • Light refreshments will be provided at each meeting. Participants can expect:
      • A safe space for sharing/discussion.
      • Support from a local pastor/spiritual leader if desired.
      • Education on meaningful and supportive topics.
      • Connection with others who understand what you’re going through.
      • Comfort in knowing you are not alone.

  • Centering Group:
    • Offered to pregnant women.
    • Be active in your pregnancy journey and prenatal care, strengthen your provider relationship, educate yourself, and build community.
    • Centering is proudly sponsored by the WV HAPI project, providing support to Appalachian families and infants through services, resources, and education.
    • Talk to the Center for Women’s Health at 304-473-2300 to learn about group availability.

  • Baby on Board Substance Use and Pregnancy Care Program (Drug Free Moms and Babies Program)
    • In partnership with the West Virginia Perinatal Partnership.
    • The Drug Free Moms and Babies Program (DFMB) is a comprehensive and integrative medical and behavioral health program for pregnant and postpartum women. The program supports healthy baby outcomes by providing prevention, early intervention, addiction treatment, and recovery support services.
