Discharge

As your health improves, we will work with you and your family to prepare for a safe homecoming.

Care Management will arrange and coordinate post-acute needs that may include inpatient rehab, skilled nursing, home care, equipment, and any specialized medications. Nursing will provide you with discharge instructions, discuss any new medications or medication changes, and inform you of follow-up appointments. The WVU Medicine Transition of Care Clinic provides patients with a bridge from Fairmont Medical Center to home when additional assistance may be needed.

Support Groups

WVU Medicine offers a variety of support services for patients and family members who are coping with a medical condition.

Supportive counseling is available for inpatients, terminal patients and their families, or any patient facing a crisis situation. Social Services also provides coordinated discharge planning services, including, but not limited to; nursing home placement, rehabilitation placement, in-home services, durable medical equipment, and transportation.

A social worker or discharge planning nurse may be contacted via phone by either the nursing or floor supervisor, should a problem, question, or crisis arise after hours.
