Patients may obtain a copy of their medical records by contacting the health information management department. Please download and complete the authorization form to release your records.

Please note: We encourage patients to request medical information at least 15 days prior to any follow-up care. Patient-related information may also be released to a physician or medical facility for follow-up care, free of charge. For other inquiries, the release of medical records may take up to 30 days and some fees may apply.
