WVU Medicine offers virtual chronic disease self-management program
MARTINSBURG, W.Va. –WVU Medicine Berkeley Medical Center and Jefferson Medical Center are now offering a virtual version of a program for those dealing with chronic disease.
Living Well Chronic Disease Self-Management Program is a six-session interactive workshop designed to help individuals coping with one or more chronic diseases learn how to take day-to-day responsibility for their care, increase the skills necessary to manage their disease, and work effectively with healthcare professionals.
This chronic disease self-management program is being offered via Zoom so participants can join from the comfort and safety of their own homes. The next session begins Nov. 17 and will be held each Monday from 5:30 - 7:30 pm through Dec. 22.
“As a full-time caregiver for my wife, the CDSMP program helped me stay organized and motivated through weekly goal setting and reflection," Al Bacchus, past participant, said of his experience with the program. “It empowered me to become a leader and give back to others facing similar challenges. My wife and I both benefitted greatly, and I’m proud to continue supporting this meaningful initiative by WVU Medicine.”
Living Well was developed by the Patient Education Research Center at the Stanford University School of Medicine. Leaders have been trained in the curriculum and follow Stanford’s guidelines which have been medically approved and rigorously tested to ensure effectiveness.
Participants of Living Well have reported significant changes in health such as less fatigue, less disability, improved functioning, and fewer hospital stays. Participants have also shown improvements in exercise adherence, pain and stress management, communication with physicians, and more confidence in their ability to manage their conditions.
Living Well is free and open to those with dealing with or caring for those with chronic conditions such as heart disease, diabetes, anxiety, depression, and chronic pain. Everyone can benefit from learning the skills to coordinate all the activities needed to manage their health, as well as help them maintain a full and active life.
