Some chronic conditions, such as chronic obstructive pulmonary disease (COPD), congestive heart failure, diabetes, and hypertension, need regular assessments to make sure you are on track and making progress with your treatment. Through remote patient monitoring, WVU Medicine can assess your health remotely while you are in your home.

After you enroll in the program, your doctor will help you decide which type of monitoring device to use. Then, we’ll send the healthcare monitoring device directly to your home and help you get it set up.

You’ll also receive our SmartHub to connect your monitoring device (not applicable for glucometers). The SmartHub device gathers all the measurements you collect and sends them to your doctor. Your measurements are automatically sent to your MyWVUChart.

Conditions We Monitor

Remote patient monitoring uses equipment and technology in your home to send information about your health to clinicians on our team. While in the program, you will have routine monitoring of vital signs and symptoms. You will also receive education about your disease on a regular basis. We engage with you and your provider to help when issues with your health arise.

Remote patient monitoring may be helpful to you if you are a WVU Medicine patient and have one of these chronic conditions:

  • Acute COVID-19
  • Atrial fibrillation
  • COPD
  • Diabetes
  • Heart failure
  • Hypertension
  • Long-haul COVID-19
  • Pneumonia
  • Sepsis

Equipment We Provide

We provide various types of equipment to patients enrolled in the remote patient monitoring program, including:

  • Blood pressure monitor
  • Glucose monitor
  • Heart rate monitor
  • Pulse oximeter
  • Scale
  • Thermometer

Cellular or Wi-Fi service is required to use our equipment. Each device has its own network connection to send the information back to your care team. We provide training on how to use each device, and we will make sure you know what to do.
