One Community
OneCommunity offers a web-based, consolidated approach to your health initiative. It’s a total population health portal unlike any other, and includes all the tools listed plus features like a personal health score and personal health tracking tools. It also features an interactive consumer engagement and robust incentives management platform.
Personal Health Score A health score is generated for each participant who completes the Personal Health Profile and biometric screening. Data is stored so health improvements can be tracked over time. While not a health diagnosis, the default health score provides an overall context for comparison within the workforce, market, and national averages.
Health Trackers: Health trackers empower your employees to monitor their progress and achieve their health goals. Trackable measurements include weight, blood pressure, cholesterol and glucose levels.
HR Insight
Internet-based financial analysis tool that allows you to project how your health care dollars will be spent. HR Insight is an internet-based financial analysis tool that allows employers to:

Personal Health Profile
Confidential information to identify unhealthy lifestyle characteristics. Confidential surveys provide the hospital with the necessary information to identify the unhealthy lifestyle characteristics and modifiable health risks of your workforce group and inform employees about their individual health risks.
Personal Health Report
Information for employees, on how to improve their personal risk factors. A customized health risk assessment designed to inform employees of their personal health risks and detail how they can achieve better health through specific lifestyle changes.

Educational Programs
Two quarterly lunch and learn seminars will be included in the program. These programs are conducted by our physicians or other clinical staff. We cover a variety of chronic conditions and other health information based on your employees’ targeted risk factors.
Monthly Wellness E-Newsletter
The program also offers the distribution of our monthly wellness e-newsletter which includes health information on acute and chronic conditions, physician spotlights, healthy recipes, news and events, and training videos.
