WVU Medicine Thomas Hospitals now offers an online records request tool that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

By voluntarily using email, you accept the risk that your protected health information may be intercepted.

Please allow 7 to 14 business days for completion of your medical records request. Note; it can take up to 30 days as allowed by law. Continuity-of-care releases are processed as priority. Please let us know if you need records for an upcoming appointment, and we will make every attempt to expedite your request. You can also have the health care provider contact the Health Information Management Department to request your records be sent directly to them in preparation of an upcoming appointment.
Records will be sent to the requester via secure email whenever possible. Records will be mailed if no email address is available. Records can be picked up by special arrangement only.
