Records Request Tool for Thomas Hospitals

WVU Medicine Thomas Hospitals offers an online records request tool that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no additional charge to use this service.

You may also request medical records through MyWVUChart, email (to [email protected]), mail (to PO Box 8049, 1 Medical Center Drive, Morgantown, WV 26506-8049), fax (to 304-598-4129) or by calling 304-598-4110 (or toll free 844-484-0304) Monday through Friday, from 8 am – 4 pm.

Use Your MyWVUChart Account

If you are enrolled in our NEW WVU Medicine Thomas Hospitals patient portal, MyWVUChart, you can request your medical records using the electronic release form within your MyWVUChart account. Once you log in, click on the health icon in the top navigation, then click on “Share My Record.” You can choose various options and download or send your medical record.

Email/Mail/Fax

Complete this authorization form to give us permission to release your medical records to you or your provider. For mental health records, please use the behavioral medicine authorization form. When you’ve completed the proper form, please email, fax, or mail the form to the WVU Medicine Enterprise Information Management department.

Email Disclaimer: The most secure way to send a Medical Records Authorization to WVU Medicine Thomas Hospitals is via fax or regular mail. If you prefer to use email to submit your authorization, this email is not secure and could be intercepted. By voluntarily using email, you accept the risk that your protected health information may be intercepted.

Processing Record Requests

Please allow 7 to 14 business days for completion of your medical records request. Please be aware that it can take up to 30 days as allowed by law. Continuing care releases are processed as priority. Please let us know if you need records for an upcoming appointment, and we will make every attempt to speed up your request. You can also have the health care provider contact the Enterprise Information Management department to request your records be sent directly to them in preparation of an upcoming appointment.

Records will be sent to the requester via secure email whenever possible. Records will be mailed if no email address is available. Records requested for personal use, attorneys, and insurance, other than for disability or workers’ compensation, will be billed prior to the release of the records. Medical records are housed for 20 years and two months, per hospital policy. For patients requesting a chart larger than 150 pages or for companies requesting healthcare records, charges apply and must be paid before records are released.

Epic health information system

As part of a coordinated care effort to help West Virginia physicians provide more efficient care to patients, WVU Medicine participates in the sharing of medical data across the state and country with any healthcare facility that uses the Epic health information system. If you change providers or move, as long as they participate in the Epic system, your provider can access your medical records via Epic. Learn more about this health information exchange.
