Wellness Screenings for Plan Year 2018 (January 1 – November 10, 2017)

Key Rewards is an incentive program that allows WVU Medicine employees to participate in wellness screenings to save money on their 2018 health insurance premiums.

All wellness screenings MUST be completed through an annual wellness/prevention visit. These visits can be scheduled anytime between January 1 – November 10, 2017 in order for biometric values to be collected. Please see more information below if you are unable to schedule an appointment with your primary care provider prior to November 10.

There are seven steps employees and covered spouses of Union employees must complete to be eligible for reduced health insurance rates (Gold Premium Rate or Silver Premium Rate):

  1. Complete an annual wellness/prevention visit with a primary care provider (1/1/17 – 12/31/17) - Click here for information on how to schedule your annual wellness/prevention visit.
  2. Receive a blood pressure screening (1/1/17 – 11/10/17)
  3. Receive a body mass index screening (1/1/17 – 11/10/17)
  4. Receive a fasting LDL cholesterol screening (1/1/17 – 11/10/17)
  5. Receive a fasting glucose screening (1/1/17 – 11/10/17)
  6. Complete a wellness consent and tobacco affidavit (3/14/17 – 11/10/17) - This can be completed by visiting MyWVUChartClick here for directions.
  7. Complete a wellness profile questionnaire online through Highmark (1/1/17 – 11/10/17) - Employees: Click here for directions. - Covered spouses of Union employees: Click here for directions. - For technical assistance: Call Highmark Web Operations at 1-877-298-3918.

To achieve the Gold premium rate, all biometric values must be within normal levels:

  • Blood Pressure less than 140/90 (both systolic and diastolic)
  • Cholesterol (LDL) less than 130
  • Glucose less than 126
  • BMI less than 26, or percent body fat less than 18 percent for males or less than 25 percent for females

Those employees and covered spouses of Union employees whose biometric values are not within normal levels will achieve the Silver premium rate and can be re-measured between January 1 – March 31, 2018 to achieve the Reasonable Alternative Standard to move from the Silver to the Gold premium rate. All re-measured biometric values must be submitted to the Wellness Center before April 1.

Please note:

  • If an annual wellness/prevention visit for employees and covered spouses of Union employees is scheduled AFTER November 10, biometric values from that visit will NOT BE AUTOMATICALLY COLLECTED. Employees and covered spouses of Union employees must contact the Wellness Center before November 10 to have biometric values collected.
  • Employees and covered spouses of Union employees who want a different documented biometric value (or values) than the most recent value (or values) automatically collected in Epic for their final values for the wellness screenings, must submit the value (or values) to the Wellness Center before November 10.
  • Employees and covered spouses of Union employees with values captured outside of WVU Medicine must submit a physical copy to the Wellness Center before November 10.
