Living Well Chronic Disease Self-Management Program is a six-week interactive workshop designed to help individuals coping with one or more chronic diseases learn to take day-to-day responsibility for their care, increase the skills necessary to manage their disease and work effectively with their health care professional.
This chronic disease self-management program begins March 26 and will be held each Thursday at 1:30 pm through May 7.
Living Well was developed by the Patient Education Research Center at the Stanford University School of Medicine. Leaders have been trained in the curriculum and follow Stanford’s guidelines which have been medically approved and rigorously tested to ensure effectiveness.
Participants of Living Well have reported significant changes in health such as less fatigue, less disability, improved functioning and fewer hospital stays. Participants have also shown improvements in exercise adherence, pain and stress management, communication with physicians and more confidence in their ability to manage their conditions.
Living Well is free and open to those with dealing with chronic conditions such as heart disease, diabetes, anxiety, depression, chronic pain… Everyone can benefit from learning the skills to coordinate all the activities needed to manage their health, as well as help them maintain a full and active life. Caregivers are also encouraged to attend.

MARTINSBURG, W.Va. –WVU Medicine Berkeley Medical Center and Jefferson Medical Center are now offering a virtual version of a program for those dealing with chronic disease.
Living Well Chronic Disease Self-Management Program is a six-session interactive workshop designed to help individuals coping with one or more chronic diseases learn how to take day-to-day responsibility for their care, increase the skills necessary to manage their disease and work effectively with healthcare professionals.
This chronic disease self-management program is being offered via Zoom so participants can join from the comfort and safety of their own homes. The next session begins Nov. 17 and will be held each Monday from 5:30 – 7:30 pm through Dec. 22.
“As a full-time caregiver for my wife, the CDSMP program helped me stay organized and motivated through weekly goal setting and reflection,” Al Bacchus, past participant, said of his experience with the program. “It empowered me to become a leader and give back to others facing similar challenges. My wife and I both benefitted greatly, and I’m proud to continue supporting this meaningful initiative by WVU Medicine.”

Living Well Chronic Disease Self-Management Program is a six-session interactive workshop designed to help individuals coping with one or more chronic diseases learn to take day-to-day responsibility for their care, increase the skills necessary to manage their disease and work effectively with their health care professional.
The goal of becoming a workshop leader is to help others learn the skills needed to manage their chronic conditions and enhance their quality of life. Previous teaching or healthcare experience is not required, but ideal candidates should have a willingness to help others improve their health. Individuals with experience dealing with a chronic condition would be ideal.
Living Well was developed by the Patient Education Research Center at the Stanford University School of Medicine. Leaders are trained in the curriculum and follow the guidelines which have been medically approved and rigorously tested to ensure effectiveness.

Leader training for Living Well will be held on Mondays, Tuesdays and Wednesdays from 1:30 – 3:30 p.m. from October 27 through December 10. It will be offered via Zoom and all leader training participants will receive a leader manual and book. Objectives of training include understanding the components of the program, learning the teaching techniques, and building the skills necessary to lead the workshops. Once trained, leaders will be asked to lead at least one workshop per year to maintain the integrity of the program.

Led by a team of expert instructors, this free 6-week series is designed to assist in developing and maintaining mindful resilience, mental fitness, and relaxation. Participants are invited to attend up to six one-hour sessions that offer a variety of simple yet practical tools to be mindful in daily living.
