BUCKHANNON, W.Va. -- The St. Joseph’s Hospital Diabetes Care Coordination Team (CCT) will host a free “Lunch and Learn” at noon on Thursday, Nov. 15, in Salvati Hall at the hospital.
The CCT is a team of healthcare professionals who work with patients and their physicians to provide better care in the home for those with type 2 diabetes. The goal is to reduce Emergency Room visits and hospital stays by providing education and visiting patients on a regular basis to help coordinate their healthcare needs.
