WVU Medicine employees and covered spouses of non-union employees are required to complete the online Wellness Profile Questionnaire by Sunday, September 30 as part of the 2018 Wellness Rewards Program.
Also here are the directions for accessing and completing the Wellness Profile questionnaire:
- Go to: https://www.highmarkbcbswv.com/home/
- Log in to your Highmark account. If you do not have an account, click Register and follow the instructions.
- Once logged in, click on Rewards Program on the right side of the page.
- In the rewards program, click Do It to take the wellness profile.
- Click Take It Now.
- If you have taken the wellness profile previously, click on Retake Wellness Profile.
- Complete all six sections of the WebMD Wellness Profile Questionnaire.
- Once you completed all sections of the Wellness Profile Questionnaire, click FINALIZE.
- You will then be prompted to a new page to answer questions about Healthy Changes, enter Contact Information, and provide feedback on this questionnaire.
- Click on FINISH. A Health Score will be calculated for you. Print this page out for your records.
You may complete the Wellness Profile online 24 hours/day, 7 days/week from any computer with internet access. The Wellness Profile will be available August 1, 2018 - September 30, 2018.
