Family Medicine recognized as Patient Centered Medical Home

MORGANTOWN, W.Va. – The WVU Medicine Clark K. Sleeth Family Medicine Center at the WVU Medicine University Town Centre Clinic was recognized as a National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) for modeling primary care that emphasizes comprehensive, accessible, patient-first care that improves the patient experience and reduces healthcare cost.  NCQA logo

The department was the first WVU Medicine practice to be recognized as a PCMH in 2014-2015 and has maintained that status.
 
“I’ve had the privilege of working at our NCQA distinguished, patient-focused practice since its first year of recognition,” Shaylee Peckens, M.D., Sleeth Family Medicine Center medical director, said. “This recognition validates our team’s continued commitment to high standards of healthcare delivery and constant improvement.”  
 
NCQA is an independent, not-for-profit organization dedicated to assessing and reporting on the quality of managed care plans, managed behavioral healthcare organizations, preferred provider organizations, new health plans, physician organizations, credentials verification organizations, disease management programs and other health-related programs. 

The NCQA Patient Centered Medical Home fosters ongoing partnerships between patients and their personal clinicians overseen by a clinician-led care team that coordinates treatment across the healthcare system. PCMH’s accomplish this through evidence-based processes that focus on highly coordinated team-based care, prevention, and ongoing communication.
