Patient and Visitor Information
Because the safety of our patients and caregivers is our highest priority, Jackson General Hospital has implemented a no-visitor policy in an effort to prevent the spread of novel coronavirus (COVID-19).
Limited exceptions will be made for end-of-life cases, pediatrics, and OB patients (women in labor). Those patients will still be limited to one visitor. Visitors who are sick will not be permitted to enter WVU Medicine hospitals under any circumstances.
Medical Records Requests
Walk-in window at Release of Information Office to close temporarily
Health Information Management’s Release of Information Office at the WVU Medicine Jackson General Hospital will be temporarily closing its walk-in services window to limit employee and customer exposure to the COVID-19 virus pandemic.
The Release of Information Office is open weekdays, 8:30 am – 4 pm.
Health Information Management thanks customers for their patience.
Please contact our Health Information Management (HIM) department between 8:30 am and 4 pm, Monday through Friday, to request medical records.
HIM can usually respond to requests for medical records within 30 days. The average turnaround time is two to three weeks.
Records cannot be provided immediately due to the time it takes to gather the requested information and process the request.
Your records may be mailed to you, or you may call ahead and make an appointment to pick them up. Medical records may also be faxed to your physicians.
How to request records:
- Your provider can request records, free of charge.
- You can call for the authorization form to receive your records by mail. The form will be sent to your address with a pre-addressed, stamped envelope. Complete the form and mail back to the medical records department.
- Make an appointment or walk in.
