Fall 2025 Cohort Admission Application
Information to gather before starting the application:
- Prerequisite (Algebra, Chemistry, English – high school or college) and general education grades (Anatomy and Physiology sequence, Nutrition, Psychology – college), if applicable
- Name(s) of all schools attended post high school including any dual enrollment credit (college coursework taken while in high school)
- Names of any/all professional certifications/licenses
Form Directions
- Extra points are not given to applicants who submit the application on August 15. It is better to take your time and fill everything out correctly.
- Give yourself at least an hour to an hour and half to complete the application. The application ranges between 45-55 questions.
- It must be completed in one setting; progress is not saved if you stop midway through it.
- Review the Application Checklist (download under Admission Requirements on https://wvumedicine.org/info/center-for-nursing-education/) for list of additional requirement materials to submit.
