Our medical staff provides every patient with outstanding, comprehensive medical treatment. However many patients and families have needs which extend past that of medical services. Our highly trained Medical Social Workers are able to help with those needs, which may include:

Discharge Planning Assistance

Recovery and rehabilitation often continue after leaving the hospital. Social Services is here to assist you and your family in coordinating the care, services and equipment you require for that process. We frequently assist patients and families in need of home health care, skilled nursing facility care, medical equipment procurement, community resource planning, and the decision making process.

Mental and Behavioral Health Assistance

Your mental and behavioral health is just as important as your medical health. For patients with mental or behavioral health needs, Social Services will work with you to strategize a plan to best address these needs. After completing a thorough assessment with the patient, arrangements can be coordinated for inpatient treatment and recovery or resources can be provided for outpatient services.

Advance Directives Counseling

Our medical staff strives to meet the patient’s medical wishes at all times and completed advance directives allow us to continue this practice should a patient no longer be able to communicate those wishes. Whether current patient or community member, Social Services is happy to review all forms of advance directives with you and assist in the completion and notary of such documents, all free of charge. Social Services is prepared to help with the following medical advance directives:

• Living Will
• Medical Power of Attorney (MPOA)
• Combined Medical Power of Attorney and Living Will
• Health Care Surrogate (Only available to current patients)
• Physician Orders for Scope of Treatment (SCOPE)
• Do Not Resuscitate Card (Orange Card)

Decision Making and Grief Counseling

While many decisions within a patient’s medical care point to an obvious resolution, Social Services is able to assist in times where such a resolution is not so obvious. Maybe a family needs guidance in when to transfer a loved one from independent or family care to assisted living or nursing care. Maybe you question whether organ donation is a good choice for you. Posed with these and many other questions, Social Services is happy to review all the options with you so you feel confident in your decisions and medical care.

In times of loss, Social Services is here to offer support and help you and your family navigate through the difficult time.

Information and Referral for Community Resources

Now that your medical needs have been managed, you may still require some assistance in managing your social needs. Social Services is able to connect you with a multitude of services that address a wide variety of social needs, including: medication assistance, housing needs, physician/provider information, transportation resources, and much more.
